We are concerned about rising medical costs and will do our best to keep
fees as low as possible by prescribing only tests, treatment, and medications
that are necessary. If you have any questions regarding your bill we would
be happy to talk with you. Please contact our insurance department to discuss
your account.
Personal
Medical Insurance
We are proud to say that our office is one of the few that bill insurance
for patients. In order to assist us in providing you this courtesy, please
be sure you bring your insurance card and/or a completed insurance claim
form with you on the day of your appointment. Payment of your deductible,
if not already met, and the patient portion of your charges is required
at the time of service.
Veterans
Administration
Please contact your Veterans Administration office for prior authorization.
You must bring a current VA authorization for each visit.
Workers'
Compensation
Please bring with you the name of the employer's workers' compensation
insurance company and date of injury. Don't forget to file an injury report
at work!
We do not routinely
bill for out-of-state worker's compensation injuries. If this is your
situation, please inform us prior to your appointment for arrangements.
Medicare
We are a Medicare participating provider, and we will submit your bill
to Medicare for you. If you also have other insurance, please bring a
completed insurance form for us to file for the balance of your charges
after Medicare makes their payment.
Medicaid
You must have a current Medicaid coupon or sticker for the month of your
visit. We will collect the Medicaid co-pay amount from you at the time
of service.
Auto
Accidents
Prompt payment of the bill is the responsibility of the person who has
received medical treatment. We will bill, as a courtesy, your personal
auto insurance. We do not bill any third party insurance or attorneys.
Arrangements can be made prior to your appointment if this is your situation.
Past
Due Accounts
Arrangements should be made with our Credit Manager for patients having
past due accounts. We want to help work with you in setting up a payment
arrangement that will help you pay your account without hurting your credit.
Remember...
It is your responsibility, not your insurance company's, to make sure
your bill is paid.
